नोटः यस फाराममा उल्लेख गरिएको जानकारी उड्डयन सुरक्षा सुधारको लागि मात्र प्रयोग गरिने छ ।

To be filled up by the Hazard observer

जोखिम पहिचान गर्ने व्यतिले भर्ने

• Any Passenger/Individual/Air Dynasty staffs identifying any hazard or any activities that may affect the safety of people and damage to equipment/aircraft/properties/vehicles are encouraged to fill up this form.
• The name of reporter shall remain confidential; however it is optional for reporter to disclose his/her identity.
• This submitted report is non-punitive and no action shall be taken against any person who discloses the safety concern, unless such disclosures indicates, beyond any reasonable doubt, gross negligence or a deliberate or willful or intentional disregard of regulations or procedures.
• सम्बन्धित कुनै यात्रु÷व्यक्ति÷एयर डाईनेष्टी कर्मचारीले व्यक्तिहरुको सुरक्षासँग सम्बन्धित जोखिम तथा उपकरण÷सम्पत्ति÷जहाज÷विमानस्थल सवारी साधनसँग सम्भावित हुनसक्ने क्षतिका गतिविधिहरुको बारेमा प्रत्यक्ष देखेमा वा सूचना पाइएमा यो फाराम भर्र्न प्रोत्साहन गरिन्छ ।
• रिपोर्ट गर्ने व्यक्तिको नाम गोप्य राखिनेछ, यद्यपि पहिचान उल्लेख गर्नु स्वैच्छिक रहेको छ ।
• उक्त पेश गरिएको रिपोर्ट गैर–दण्डनीय हुनेछ र सुरक्षा सम्बन्धी जानकारी खुलाएमा सजाय दिने कार्य हुने छैन तर त्यस्ता खुलासाले कुनै उचित शंका, घोर लापारवाही वा नियम र प्रक्रियाहरुको जानाजानी बेवास्ता गरी उल्लंघन गरेको संकेत पाइएमा गैर–दण्डनीय नीति लागू नहुने जानकारी गराउँछौ ।
